Chapter 688 provides transition services for eligible HIGH SCHOOL STUDENTS WITH SEVERE DISABILITIES.
Discuss this at your IEP Meetings because the school must submit the referral within 2 years prior to graduation (or before turning 22). In order to be eligible, a student must (in summary):
-already be receiving special ed services; -need continued services; and -have job related goals but is unable to work without support, and for more than 20 hours a week. (Receiving SSI and SSDI income, or being registered with the Mass Comm. for the Blind automatically qualifies a student for Chapter 688 transition services.
